If you consider your loss to be serious, or if it has involved damage from water, please call 1-800-648-1794 to report it directly to an attendant who will have an adjuster contact you as quickly as possible.

If you prefer to report your incident via this Loss Notice Form, please note that it will NOT be reviewed until the beginning of the next business day if submitted after business hours (Monday - Friday 9:00 am - 4:00 pm)
